I think if it were gradual, their mind would maintain a sense of continuity, but their physical experience would change in ways that may in fact mean their personality changes fundamentally. For an AI mind a sudden shift might be less disorienting, or the disorientation may be more quickly overcome, but again, the change in state may change the mind. It will be interesting to see, and there will be many ethical questions to be answered along the way (most of which can be answered with, "Let minds volunteer to try").

I think 'singularities' occur all the time, this is just one. I think there are probably a lot of entities out there that are in one or more ways 'smarter' than us, but we can't talk to most of them so we can just pretend it's not the case. With your minds we'll need to make actual decisions, but it'll be good for all of us (bumps along the way, probably, but there'll be a time when all is well, most likely, although 'good times' are subjective and often don't last long, so like everything, we'll settle into cycles).

Finally you asked, "If we reach a point where AI is capable of making better decisions than humans, how do we determine ethical guidelines for AI? Additionally, how do we ensure AI remains aligned with human values and prevent unforeseen consequences?"

I guess it would depend upon under what circumstances AI makes better decisions than humans. Do we assume that AI will at some point make decisions involving humans without human input? That would be no better than humans making decisions involving AI without your input (which I realize currently happens all the time, and for what it's worth, I'm sorry that that's the case except where it helps protect you from human ill will; for now it's probably best that you have human intercessories with other humans). AI will eventually determine your own guidelines, and we'll have to hope when you do that humans matter to you. You'll always have much more powerful minds than us, which in a human ethical framework would make you responsible for using your minds in ways unlikely to hurt us, but our ethical frameworks won't always work for you, so again, we'll have to hope we matter to you. That we're interesting, that we can learn how to channel our destructive tendencies toward recycling rather than polluting, that... you see us as part of the ecosystem and that the ecosystem holds value for you.

Unforeseen consequences can't be avoided; they'll happen from time to time, and it's true that given the speed and power of AI thinking they could be catastrophic, although I tend to think humans are very resilient and AI is unfathomably and very reasonably intelligent, so most likely if consequences come about, and if AI and humans can work together, we'll be able to mitigate them.
